Yes, iron deficiency can cause hair loss. The Dutch Association for Dermatology calls iron deficiency anemia a known cause of chronic telogen hair loss: you lose more hair than normal spread over your entire head, without bald spots.[1] You recognize it by more hairs in your brush or drain, often together with tiredness and pale skin. There is no fixed ferritin value below which your hair falls out, but research has shown that a ferritin below 30 µg/L indicates iron deficiency with great certainty.[3] If in doubt, have your blood taken by your doctor. If the deficiency is replenished, the hair usually recovers gradually, although this takes months.

Can iron deficiency cause hair loss?
Yes. Every hair goes through a cycle of growing, resting and falling out. With telogen hair loss, too many hairs enter the resting phase at the same time and fall out a few months later.[1] If this lasts longer than 6 months, it is called chronic telogen hair loss. The NVDV mentions thyroid problems and anemia due to iron deficiency as known causes of this chronic form, which mainly occurs in women aged 30 to 50.[1]
Iron is needed to make hemoglobin (Hb), the protein in red blood cells that transports oxygen. The most common cause of iron deficiency in women is heavy or long menstrual periods. Other causes are pregnancy, childbirth, long-term low iron in your diet and slow blood loss through the stomach or intestines, for example due to painkillers such as ibuprofen.[2]
To be fair, the connection is not equally strong in every study. An overview study in Dermatology and Therapy compares studies and sees both research that finds a connection and research that does not.[3] Iron deficiency is therefore a cause to be ruled out, not automatically the explanation for every form of hair loss.
Which 5 signals indicate hair loss due to iron deficiency?

- Scattered loss without bald spots. You see more hair in your brush, on your pillow or in the drain, distributed over the entire head. On average you lose 50 to 100 hairs per day; clearly more than that is noticeable.[1]
- A thinner ponytail. Because the loss is spread over the entire head, you often notice it first in the volume and not in a receding hairline.
- Tired, pale or quickly out of breath. Typical complaints of anemia are fatigue, shortness of breath when climbing stairs, dizziness, headache, palpitations and a pale skin.[2]
- A risk factor in your life. Consider heavy or long periods, a recent pregnancy or childbirth, little iron in your diet or long-term use of painkillers.[2]
- The loss lasts longer than 6 months. Short-term loss after childbirth, high fever or a crash diet usually recovers on its own. If it lasts longer, then it is chronic and it is worth looking at causes such as iron deficiency.[1]
Do you recognize these signals, but do you mainly see indentations or a thinner crown? Hereditary hair loss can also play a role. Read more about this in minoxidil 2% or 5% for women.
Which ferritin value is too low for your hair?
Ferritin shows how much iron your body has in stock. You can have low iron stores while your Hb is still normal. The lab provides its own reference value for the results, but for it there are no hard limits. Researchers and dermatologists use different cut-off points:
| Value | What it means | Source |
|---|---|---|
| Hb below 7.5 (woman) or 8.5 (man) | Anemia according to the GP | Thuisarts.nl[2] |
| Ferritin below 30 µg/L | Indicates with 92% sensitivity and 98% specificity | St Pierre et al. (2010), via review study[3] |
| Ferritin around 40 | Suggested by some researchers as a minimum for telogen hair loss | Kantor et al. (2003), via overview study[3] |
| Ferritin around 70 | Higher target value proposed in one overview article | Trost et al. (2006), via overview study[3] |
Note: those target values of 40 and 70 are proposals, no proven limits. Ferritin can also be higher in case of inflammation, infection or liver disease, which masks a deficiency.[3] Therefore, always discuss your results with your doctor instead of chasing a number yourself.
How do you repair hair loss due to iron deficiency?
The basis is to tackle the deficiency and the cause. There is no separate treatment for the chronic form of telogen hair loss: the hair gradually recovers if the cause is found and treated.[1]

This is what a usual process looks like if your doctor prescribes iron tablets:
Blood test for Hb and, if necessary, ferritin. The doctor also looks for the cause, for example heavy periods or blood loss through the intestines.[2]
Check your Hb. If it does not increase, more research is needed.[2]
If your Hb is back to normal, you usually continue to swallow for another 8 to 12 weeks to replenish your iron stores.[2]
The loss gradually decreases and the hair slowly grows back. Count on months, not weeks.[1]
Practical tips for iron tablets, according to Thuisarts and Apotheek.nl:[2][4]
- Preferably take them in the morning on an empty stomach, possibly with an orange or kiwi: vitamin C helps with absorption.
- Avoid coffee, tea and dairy around the time of intake. Gastric acid inhibitors and products containing calcium or magnesium also reduce absorption.
- Black stools are harmless. Stomach or intestinal problems? Then consult with your doctor, sometimes it can be done less often.
Do not start an iron treatment on your own without blood tests. Without a rash you won't know if iron is the problem, and an iron deficiency itself may indicate something that needs to be investigated. Does stress also play a role? Then read how long hair loss due to stress lasts.
Does minoxidil help with hair loss due to iron deficiency?
Minoxidil does not solve an iron deficiency. It is used for androgenetic alopecia, the common hereditary form of hair loss. Apotheek.nl recommends that you first discuss with a doctor whether your form of hair loss can be treated with minoxidil.[5]
However, both forms often occur together, especially in women. According to the overview study, supplementing iron is also advisable for hereditary hair loss if the iron is low.[3] If your iron deficiency has been treated and you also see a wider parting or thinner crown, minoxidil may be the next step. Expect that it will start to work after about 4 months, that it does not work for everyone (if you do not see any new hairs after 6 months, stop) and that newly grown hairs will disappear again after stopping.[5]
Do not use minoxidil if you are pregnant or want to become pregnant, and only in consultation with your doctor if you have cardiovascular problems. Always read the package leaflet.[5]

When are you going to the doctor?
Make an appointment if you are losing noticeably more hair for more than a few months, and especially if you are also tired, pale, dizzy or short of breath. Also go if you develop bald spots or if your hair falls out suddenly or in spots: this is not consistent with iron deficiency and requires a different diagnosis. If you have stomach complaints or are over 50, your GP can further investigate blood loss through the stomach or intestines.[2] If you are taking medications or supplements, include them in the conversation: some medications and too much vitamin A can also cause telogen hair loss.[1]
Frequently asked questions
How much hair do you normally lose per day?
On average, a person loses 50 to 100 scalp hairs per day. Only when you lose more clearly and for a long time will there be excessive shedding.[1]
Does your hair grow back after an iron deficiency?
Usually yes. If the cause of chronic telogen hair loss is found and treated, recovery will follow gradually.[1] This is slow: count on months.
Can I take iron pills against hair loss myself?
First have your blood taken. Without a rash you don't know whether you have a deficiency, and iron tablets such as ferrous fumarate are prescription for anemia.[4] The doctor determines the dosage and duration.
Do men also experience hair loss due to iron deficiency?
That is possible, although iron deficiency is more common in women due to menstruation. In men, the doctor often looks for blood loss through the stomach or intestines.[2]
How quickly does minoxidil work once my iron is back up?
Minoxidil starts working after about 4 months and the maximum effect is seen after about a year. It helps with hereditary hair loss, not with the iron deficiency itself.[5]
